Welcome to Humulin R U-500

Humulin R U-500 is an insulin that is used to control high blood sugar in adults and children with diabetes mellitus who need more than 200 units of insulin in a day.

Humulin R U-500 contains 5 times as much insulin in the same volume (500 units/mL) as Humulin R U-100 (100 units/mL).
It is not known if Humulin R U-500 is safe and effective when used in combination with other insulins or when used in an insulin pump.

Meaningful results, simpler regimen*†

In a clinical study, U-500 was shown to lower A1C by an average of 1.1% in people who were not well controlled when taking more than 200 units of standard U-100 insulin a day.‡

U-500 is an insulin that is taken 2 or 3 times per day. Because it can be prescribed without other insulins, you may be able to reduce the number of insulins you take.

*Compared to standard U-100 insulin. Most patients will require 2 to 3 daily injections of U-500. U-500 KwikPen dials and doses the units you need, up to 300 units at a time.

†With U-500, you can inject up to 80% less liquid and still get the dose you need.

‡The purpose of the study was to compare the efficacy and safety of 2 dosing treatments (2 or 3 injections per day) of U-500 in adults with type 2 diabetes who were uncontrolled with high doses of standard U-100 insulin. The results between the 2 treatments were similar.

24-week, open-label, randomized trial to compare the efficacy and safety of 2 dosing regimens (TID, n=162 vs BID, n=163) for U-500 insulin replacing high dose U-100 insulin (>200 units per day) with or without oral antihyperglycemic drugs in adult patients with uncontrolled type 2 diabetes. These regimens were found to be equivalent for A1C reduction over 24 weeks, and both were efficacious.1

SELECT SAFETY INFORMATION

  • Do NOT perform dose conversion when using the Humulin R U-500 KwikPen or U-500 insulin syringe.
  • Do not take Humulin R U-500 if your blood sugar is too low (hypoglycemia) or you are allergic to any of the ingredients in Humulin R U-500.

Humulin R U-500 is a concentrated insulin

U-500 is 5 times more concentrated than standard U-100 insulin. When insulin is concentrated, it takes less liquid per dose than standard U-100 insulin to help reduce your blood sugar levels.

With U-500, you can inject up to 80% less liquid and still get the dose you need. Reducing the amount of liquid may also reduce the number of times you have to inject in a day.* And because U-500 can be prescribed by itself, you may be able to reduce the number of insulins you take.

About the U-500 KwikPen®

With the U-500 KwikPen, you may notice some differences:

If not, they can order them for you. Once ordered, it typically takes 1 to 2 business days for the pharmacy to receive the medication and syringes.

Narrator: When you’re ready, pull the Pen cap straight off. Do not remove the Pen label. Wipe the rubber seal with an alcohol swab. The insulin should be clear and colorless. Do not use if it is cloudy, colored, or has particles or clumps in it.

Narrator: Continue holding your pen with the needle pointing up. Push the dose knob in until it stops, and zero is seen in the dose window. Hold the dose knob in and count to 5 slowly. You should see insulin at the tip of the needle. If you do not see insulin, repeat the priming steps, but not more than 8 times. Narrator: Humulin R U-500 KwikPen has been designed to deliver the dose in insulin units that is shown in the dose window. Ask your doctor what your dose should be for this Pen. You can give from 5 to 300 units in a single injection. If your dose is more than 300 units, you will need to give more than one injection. If you need help deciding how to divide up your dose, ask your doctor. You must use a new needle for each injection and repeat the priming step.

Narrator: To select your dose, turn the dose knob to select the number of units you need to inject. The dose indicator should line up with your dose. The dose knob clicks as you turn it. Each click of the dose knob dials 5 units at a time. Do not dial your dose by counting the clicks because you may dial the wrong dose. This may lead to you getting too much insulin or not enough insulin. The dose can be corrected by turning the dose knob in either direction until the correct dose lines up with the dose indicator. The even numbers (for example, 10, 20, 30) are printed on the dial. The odd numbers (for example, 5, 15, 25) are shown as lines between the even numbers. Always check the number in the dose window to make sure you have dialed the correct dose. The Pen will not let you dial more than the number of units left in the Pen.

Narrator: First, wipe the skin with an alcohol swab and let the injection site dry before injecting. Then, insert the needle into your skin. Push the dose knob until it stops. Hold the dose knob in and slowly count to 5. The plunger only moves a little with each injection, and you may not notice that it moves. Do not try to inject your insulin by turning the dose knob. You will not receive your insulin by turning the dose knob.

Narrator: After counting to 5, pull the needle out of your skin. A drop of insulin at the needle tip is normal. It will not affect your dose. If you see zero in the dose window, you have received your full dose. If you do not see zero in the dose window, you did not receive your full dose. Do not redial. Narrator: Carefully replace the outer needle shield. Unscrew the capped needle and throw it away. Do not store the Pen with the needle attached to prevent leaking, blocking of the needle, and air from entering the pen. Replace the Pen cap by lining up the cap clip with the dose indicator and pushing it straight on.

Narrator: Put your used needles in an FDA-cleared sharps disposal container right away after use. Do not throw away or dispose of loose needles in your household trash. If you do not have an FDA-cleared sharps disposal container, you may use a household container that:

  • is made of a heavy-duty plastic,
  • can be closed with a tight-fitting, puncture-resistant lid, without sharps being able to come out,
  • stands upright and is stable during use,
  • is leak-resistant, and
  • is properly labeled to warn of hazardous waste inside the container

When your sharps disposal container is almost full, you will need to follow your community guidelines for the right way to dispose of your sharps disposal container. There may be state or local laws about how you should throw away used needles and syringes. For more information about safe sharps disposal, and for specific information about sharps disposal in the state that you live in, go to the FDA’s website at: www.fda.gov/safesharpsdisposal. Do not dispose of your used sharps disposal container in your household trash unless your community guidelines permit this. Do not recycle your used sharps disposal container. The used Pen may be discarded in your household trash after you have removed the needle.

Humulin® (HUE-mu-lin) R U-500 (500 units/mL) is for adults and children who need more than 200 units of insulin in a day to control high blood sugar for their diabetes mellitus. It is more concentrated than Humulin R U-100. It has 5 times as much insulin in each mL as Humulin R U-100 (100 units/mL).

It is not known if Humulin R U-500 is safe and effective when used with other insulins or when used in an insulin pump.

Warnings: Humulin R U-500 may cause serious side effects, including:

Severe low blood sugar, which can lead to seizures, unconsciousness, and death.

Severe allergic reactions. Get medical help right away if you develop a rash over your whole body, have trouble breathing, have a fast heartbeat, or are sweating.

Swelling of your hands and feet. Tell your doctor if you are short of breath, have swelling in your ankles, or have gained weight suddenly.

Heart failure when taking a medication from a class of drugs called thiazolidinediones (TZDs) with Humulin R U-500. This may occur in some people even if they have not had heart problems before.

Low potassium in your blood (hypokalemia). This can lead to severe breathing problems, irregular heartbeat, and death.

Do not share your Humulin R U-500 KwikPen® or U-500 syringe with anyone. Even if you have changed the needle, you or the other person can get a serious infection.

When using the Humulin R U-500 KwikPen: The Humulin R U-500 KwikPen is made to dial and deliver the correct dose of Humulin R U-500 insulin. Do not remove Humulin R U-500 from the KwikPen to inject with any syringe. This could cause severe overdose and may lead to death.

When using the Humulin R U-500 vial: There is a special syringe to measure Humulin R U-500 called the “U-500 insulin syringe.” Only use the U-500 insulin syringe to inject Humulin R U-500. If you do not use the right syringe, you may take the wrong dose of Humulin R U-500. This could cause severe overdose and may lead to death.

Do NOT perform dose conversion when using the Humulin R U-500 KwikPen or U-500 insulin syringe.

Do not use Humulin R U-500 in an insulin pump or inject it into your vein.

Do not take this medicine if you have low blood sugar.

Common side effects

The most common side effects of Humulin R U-500 include:

Allergic reactions, such as redness and swelling at the site where you inject.

Skin thickening or pits at the injection site (lipodystrophy).

Itching and rash.

All the medicines you take, especially a class of drugs called thiazolidinediones, or TZDs. Be sure to include the over-the-counter medicines, vitamins, and herbal supplements you take.

How to take

Read the instructions that come with your Humulin R U-500 carefully. Take it exactly the way your doctor tells you.

Know how much Humulin R U-500 you are supposed to take. Do not change your dose unless your doctor tells you to.

Check the label of your insulin each time you use it. This will help you make sure you are using the right one.

Test your blood sugar before you take Humulin R U-500. Do not take it if your blood sugar is too low.

Do not mix Humulin R U-500 with any other insulin.

Always use a new needle when injecting Humulin R U-500. This will help you avoid infection.

Inject Humulin R U-500 under your skin. Change (rotate) where you inject your insulin with each dose. Do not inject your insulin into the exact same spot. Avoid injecting your insulin into areas where the skin has pits or lumps, or is thickened, tender, bruised, scaly, hard, scarred, or damaged. This will help reduce your chance of getting pits, lumps, or thickened skin where you inject your insulin.

Dosing and storing Humulin R U-500

Where to inject U-500 insulin. Injection sites include the stomach area, buttocks, upper legs, and upper arms.
  • Whether you use the U-500 KwikPen or a U-500 vial and U-500 syringe, you'll want to rotate the site each time you inject. Always be sure to wipe your skin with an alcohol swab and let the area dry first.
  • Confirm your dose with your doctor. For a handy reference, please have your doctor write down your daily dosing schedule.
  • Administer Humulin R U-500 subcutaneously two or three times daily 30 minutes before a meal into the thigh, upper arm, abdomen, or buttocks.
  • Be sure to refer to the Instructions for Use that came with your U-500 KwikPen or vial. Of course, if you have any questions, please be sure to talk to your doctor.
  • How should I store Humulin R U-500? Keep Humulin R U-500 away from heat and direct sunlight and out of the reach of children. An unopened pen or vial should be kept in the refrigerator and discarded after the expiration date stamped on the label.
  • Storing the Humulin R U-500 KwikPen: Once opened, do not refrigerate the Humulin R U-500 KwikPen. Throw away the pen you are using after 28 days, even if there is still insulin left.*
  • Storing the Humulin R U-500 vial: Do not use if it has been frozen. An unopened vial should be kept in the refrigerator until the expiration date. An open vial can be stored at room temperature and must be thrown away within 40 days of opening even if insulin remains.**

  • Humulin R U-500 KwikPen: The U-500 KwikPen is specially made to dial and deliver doses of Humulin R U-500 insulin. Do not use any syringe to remove Humulin R U-500 insulin from your U-500 KwikPen. The markings on certain syringes will not measure your dose correctly. A severe overdose can happen, causing low blood sugar, which may put your life in danger.
  • Humulin R U-500 vial: There is a special U-500 insulin syringe to measure Humulin R U-500. Use only a U-500 insulin syringe to draw up and inject your Humulin R U-500. If you do not use the right syringe type, you may take the wrong dose of Humulin R U-500. This can cause you to have too low blood sugar (hypoglycemia) or too high blood sugar (hyperglycemia). Your healthcare provider should show you how to draw up Humulin R U-500.

What to expect

To know if U-500 is helping to improve your glucose, check your blood sugar.

Low blood sugar is the most common side effect of U-500.

It's normal to want to understand possible side effects like low blood sugar when starting a new insulin. Be sure to talk to your doctor about the causes and how to identify and manage low blood sugar.

Take your dose 30 minutes before eating a meal as your doctor instructed. Timing is important to help reduce the risk of low blood sugar (hypoglycemia).

As with any insulin, there is potential to experience low blood sugar at any time, including in the first few weeks as you're getting used to your new medication or dose. Checking your blood sugar as directed by your doctor will help monitor for any highs or lows, as well as possible improvements in your blood sugar.

What is Humulin® R U-100?

Humulin R U-100 is a short-acting insulin, which means it can cover insulin needs for meals eaten within 30 minutes.

There are 2 types of Humulin R insulin: Humulin R U-100 and a more concentrated insulin, Humulin R U-500. U-100 insulins, including Humulin R, contain 100 units per mL. It's important to make sure you have the correct one.

Humulin R U-100 is for injection under the skin. It should not be injected into the muscle. Doses of insulin are measured in units. With Humulin R U-100, it is important to use a syringe that is marked for U-100 insulins.

What is Humulin® N?

Intermediate-acting insulin to cover blood sugar increases between meals and during the night

Humulin N is an intermediate acting insulin that is slower to act and lasts longer than regular human insulin. Humulin N is available in the prefilled Humulin® N KwikPen® or in a Humulin N U-100 vial.

Humulin N is injected under the skin. It should not be injected into muscle or veins. Doses of insulin are measured in units. U-100 insulins, including Humulin N, contain 100 units/mL. With the Humulin N U-100 vial, it is important to use a syringe that is marked for U-100 insulin preparations.

Do not use Humulin N:

  • If the insulin substance (the white material) remains at the bottom of the bottle after mixing, or
  • If there are clumps in the insulin after mixing

INDICATION

Humulin® (HUE-mu-lin) R U-500 (500 units/mL) is for adults and children who need more than 200 units of insulin in a day to control high blood sugar for their diabetes mellitus. It is more concentrated than Humulin R U-100. It has 5 times as much insulin in each mL as Humulin R U-100 (100 units/mL).

It is not known if Humulin R U-500 is safe and effective when used with other insulins or when used in an insulin pump.

SAFETY SUMMARY

Warnings -

Humulin R U-500 may cause serious side effects, including:

  • Severe low blood sugar, which can lead to seizures, unconsciousness, and death.
  • Severe allergic reactions. Get medical help right away if you develop a rash over your whole body, have trouble breathing, have a fast heartbeat, or are sweating.
  • Swelling of your hands and feet. Tell your doctor if you are short of breath, have swelling in your ankles, or have gained weight suddenly.
  • Heart failure when taking a medication from a class of drugs called thiazolidinediones (TZDs) with Humulin R U-500. This may occur in some people even if they have not had heart problems before.
  • Low potassium in your blood (hypokalemia). This can lead to severe breathing problems, irregular heartbeat, and death.

Do not share your Humulin R U-500 KwikPen® or U-500 syringe with anyone. Even if you have changed the needle, you or the other person can get a serious infection.

When using the Humulin R U-500 KwikPen: The Humulin R U-500 KwikPen is made to dial and deliver the correct dose of Humulin R U-500 insulin. Do not remove Humulin R U-500 from the KwikPen to inject with any syringe. This could cause severe overdose and may lead to death.

When using the Humulin R U-500 vial: There is a special syringe to measure Humulin R U-500 called the “U-500 insulin syringe.” Only use the U-500 insulin syringe to inject Humulin R U-500. If you do not use the right syringe, you may take the wrong dose of Humulin R U-500. This could cause severe overdose and may lead to death.

Do NOT perform dose conversion when using the Humulin R U-500 KwikPen or U-500 insulin syringe.

Do not use Humulin R U-500 in an insulin pump or inject it into your vein.

Do not take this medicine if you have low blood sugar.

Do not change the insulin you use without talking to your doctor. Changing insulin may lead to low or high blood sugar.

Do not drive or use heavy machinery until you know how Humulin R U-500 affects you. Do not drink alcohol while using Humulin R U-500.
